Claims for reimbursement can be filed by either logging in to your participants online Participant Portal (https://adminamerica.lh1ondemand.com/Login.aspx?ReturnUrl=%2f) and clicking on the blue Reimburse Myself box under the home tab or by completing a claim form (https://adminamerica.com/wp-content/uploads/2019/08/FSA-Reimbursement-Documentation-Form.pdf) and returning it to the one of the addresses or fax number noted on the bottom of the claim form with a copy of a detailed statement for the expenses incurred.
